How to earn recertification credit

Contact Melissa Mallinson at the beginning of the session in which you're taking a class, workshop, or private lesson. Make sure your instructor knows you're trying to get the CPDUs, too. If s/he looks confused, you may need to explain it to them, or just have them call me.
You must attend at least 80% of the dates class is held. CPDUs will be given only for those hours you attend class. It is the instructor's right to decide if you have not attended enough of the classes to satisfy learning the curriculum. That is, attendance is not enough to satisfy the requirement; you must learn something, obviously.
You must complete two state forms available from Ari or online: ISBE 77-21A (Evaluation of Workshop, Conference, Seminar, etc.) and ISBE 77-21B (Evidence of Completion Form).
You must also complete a form for the Old Town School.
Then just sit back and relax and I'll submit the required forms to the state. You can use the information below to contact me and return forms. Please note that some of these forms require your signature, so they cannot be returned to me online. I must have a hard copy with your signature. Thanks!
